Accredited Hospices of America is seeking an uncommon individual
to be the
case
manager and principle point of contact for the
family
members,
social workers,
chaplains, and other healthcare professionals
that provide loving care for their
patients with life ending illnesses.
This position is much more than just a job. You won't report to a facility; rather, you'll
provide care in the patient's home environment. You'll need to be on round-the-clock
call for one week each month to cover any possible emergencies.
We create tailored care plans to meet patient needs. As a premiere hospice provider
in our community, we offer the most comprehensive hospice and bereavement care
available in the greater Houston area.
This position isn't for everyone, but for the right person, it offers a rich, rewarding work
experience and the satisfaction of helping patients in their final stage preparations. We
have an experienced staff that includes physicians, compassionate nursing assistants,
social workers, chaplains and specially trained volunteers. We partner with you to offer
full-spectrum hospice care staff who are on-call 24 hours a day, 7 days a week, every
day of the year.
Our Medical Directors have extensive experience in Hospice and Pallative Care, and
are available for consultation regarding hospice, palliative, and bereavement care.
They are very good at maintaining clear, effective, ongoing communication, and they
serve as a resource to which you may refer patients and families as well.

Requirements:
Active nursing license (RN)
Valid Texas driver's license
Dependable car with current insurance
Military experience valuable, but not necessary
